COVID Vaccine Availability

The Village of Hazel Crest will be offering COVID vaccines for individuals 65 and older on Wednesday, February 3, 2021
between 10:00 a.m. – 3:00 p.m.
at Village of Hazel Crest
(3601 183rd Street, Hazel Crest, IL)

VACCINES ARE BY APPOINTMENT ONLY

To schedule an appointment, you must email ccarter@villageofhazelcrest.com and include your name, address, DOB and phone number.

You will be contacted via email with a specific appointment time.
